Getting to Measures That Matter

“Promoting effective communication and coordination of care” is one of NQS' six priorities. Successful care coordination has been associated with higher-quality care, improved efficiency, better patient experience, and reduced costs. With the majority of one’s health and well-being determined by factors outside the healthcare system, there is a need to broaden the scope of care coordination and the performance measurement associated with monitoring progress.

This project will seek to understand and recommend performance measurement approaches for the “health neighborhood,” a dynamic environment of coordination between providers of primary care and community-based services. This effort will leverage new approaches to sharing data, facilitate the integration of population health and primary care, and support ongoing efforts to improve care coordination.
