Comments on the Common Formats Version 1.0 


The comment period for Common Formats, Version 1.0, closed on December 31, 2009. Thank you for your submitted comments.

NQF, on behalf of the Agency for Healthcare Research and Quality (AHRQ), coordinated a process to obtain comments from stakeholders about the Common Formats authorized by the Patient Safety and Quality Improvement Act of 2005 (The Patient Safety Act). The Common Formats provide a standardized method for data collection and reporting of patient safety event information to Federally-listed Patient Safety Organizations (PSOs).

A commenting tool was made available during the period, which allowed a user to comment on the Common Formats. This tool was not intended to allow submission of actual patient safety events. As an example, for the item "Documentation of whether the fall was observed" -- the user was allowed to comment on the content and wording of the item, but the user was not permitted to enter comments using patient information.

Collection of comments on Version 1.0 closed as of December 31, 2009. An initial set of comments, collected during the 60 days immediately after release of Version 1.0, were provided to AHRQ to consider as AHRQ develops the technical specifications for the Common Formats Version 1.0. Thereafter comments were reported to AHRQ at a frequency warranted by the volume of comments received. Users of this site will be advised of posting of new versions of the Common Formats as they are released. 

Specifically, a user was permitted to comment on the Common Formats in their entirety or comment on any item of the event descriptions. A user could have commented on any part of the Common Formats event description displayed on the screen simply by selecting it. Also, a user was allowed to comment on item(s) on displayed sections of the reporting form, as well as comment on the instructions given for each event description. All feedback is valuable; therefore, we thank all those who participated in the comment period.
