Overview
The Critical Paths: Care Coordination project will assess the readiness of electronic data and health IT systems to support data capture, normalization and standardization to support care coordination reporting and evaluation across clinical information systems (e.g., electronic health records (EHRs) health information exchanges (HIEs), etc.). This project will assess the readiness of electronic data to support measurement concepts related to care coordination. The scope is focused on transitions of care and communication of the patient plan of care.
This project will assess the ability of existing health IT infrastructure (Quality Data Model (QDM), Healthcare Quality Measure Format (HQMF) , Measure Authoring Tool , electronic health records (EHRs)) to support the use of medical device data for purposes of quality measurement and reporting. The future state is to enable the use of care plan data communicated during transitions of care (e.g., problems/diagnoses, interventions /orders, and expected outcomes/goals) for quality measurement.
Through conference calls and an in-person meeting, the Care Coordination Technical Expert Panel (TEP) is defining the data elements and processes of care associated with care coordination. A report with the needed data elements will be completed by June 1, 2012.

The in-person meeting will be held April 12-13, 2012.
An RFP for a qualified contractor to perform the Environment Analysis was posted on the NQF website March 1- April 6, 2012. The contract will be awarded by May 4, 2012.
Based on the data elements defined by the TEP, the environmental analysis will be completed by August 10, 2012.
Critical Paths Care Coordination Environmental Scan Report
The TEP will review the results from the environmental analysis and develop a draft report for the measurement concept area that highlights the gaps and provides actionable steps to address them. The draft report will be posted on NQF's website for public comment, and a webinar will be held during the commenting period to review the report and encourage public comments. The posting of the draft report is projected for early October 2012.

This webinar reviewed the work of the Technical Expert Panel to define requirements for measurement, discuss the results from an environmental scan, and introduce a draft recommendations report to advance data readiness. The draft report is open for public comment through October 30, 2012.
