Despite overall improvements in the healthcare system, disparities based on social risk factors continue to persist. This is the first of three interim reports that will culminate in a final report which will be released in September 2017. This interim report describes a review of the literature on disparities in health and healthcare related to the target conditions listed above based on the social risk factors identified in the NAM report, Accounting for Social Risk Factors in Medicare Payment: Identifying Social Risk Factors. The findings from this interim report will inform the first phase of the Committee’s work to develop a roadmap and conceptual framework for how performance measurement can be leveraged to eliminate disparities in health and healthcare.
