Abstract
In its 2008 report, National Priorities & Goals—Aligning Our Efforts to Transform America’s Healthcare, the National Priorities Partnership (NPP) identified six National Priorities that if addressed would significantly improve the quality of healthcare delivered to Americans. In an effort to look beyond the walls of the healthcare delivery system, NPP identified Population Health as one of the six Priority areas. In February 2010, NPP convened the Population Health Workgroup to establish specific actions for NPP partners and others to consider—actions that if implemented would have the greatest potential to meet the three overarching Population Health Goals.
